How do I create a Reporter intake form?

To create a Reporter intake form:

  1. You must have an option list(s) and field(s) set up in advance; go to How do I create an option list? and How do I create a field? to learn how

  2. After you have created your option list(s) and field(s), go to “Forms” on the menu and go to “Reporter intake forms”

  3. Click on “New Reporter Intake Form” on the top right corner of the page Screenshot 2025-02-11 at 14.39.25

  4. Enter the name of your Reporter intake form Screenshot 2025-02-11 at 14.46.25

  5. Click into your Reporter intake form

  6. Click on “Add fields to intake form” Screenshot 2025-02-11 at 15.12.11

  7. Select the field(s) you wish to ask to the reporters Screenshot 2025-02-11 at 15.13.25
  8. Click on “Add to form”

Additional possibilities:

You can make a specific field mandatory, or remove a field, or add conditional fields (make a second question only appear depending on the first answer).

  1. Go to “Forms” on the menu and go to “Reporter intake forms”

  2. Click into the relevant Reporter intake form and click into the relevant field

  3. A side window will appear on the right, where you can:

    1. Switch on “Mandatory field” to make a field mandatory
    2. Click the button “Manage Conditions” to add conditional fields - for further instructions, visit How do I manage conditions between fields in my Reporter Intake Form?
    3. Click on “Remove field from form” to remove the field

  • If you are going to enable multiple reporting languages in your SpeakUp system and decide to add a short text field to a Reporter intake form, a Reporter’s responses will go through translation. Within an issue, you will able to select between the Reporter language and client default language, to see both the original and translated response for the short text field.
  • To add a Reporter intake form to a Channel, go to How do I add a Reporter intake form to a channel?.  If you don’t add the form to a Channel, it will not be in use.
