How do I create an Issue form?
To create an Issue form

Go to “Forms” on the left hand menu, and click on “Issue forms”

Click on “New issue form” on the top right corner of the page

Insert a name for your form and click on “Save”

Click into your new form and click on “Add fields to form”
- Select the relevant fields and click on “Add to form”
- Use the two horizontal bars to drag and drop and to reorder your fields
