How do I create an Issue form?
To create an Issue form
- Click on the Settings icon located in the top-right corner of the page.

Go to “Forms” on the left hand menu and select “Issue forms”

Click on “New issue form” on the top right corner of the page

Insert a name for your form and click on “Save”

Select your new form and click on “Add fields to form”
- Select the relevant fields and click on “Add to form”
- Use the two horizontal bars to drag and drop and to reorder your fields
- You can adjust the Form usage to allow the form to be completed once per issue or multiple times
- You can also define whether the form is assigned automatically when a new issue is created, or added manually after that
